Textbox in radio buttons

Hi,

I want to make a radio buttons with a text box in one of the options. It should have the following structure:

Do you want this?
– Yes, namely <text box> items
– No

I want the text box (or number box more specifically) to appear in only of the options, without having it to be a separate question. Is this possible?
