I tested this again and I can confirm that it works the same for me:
– if I just enter the page directly – the issue is there
– if I navigate to another page – the form shows correctly
– even if I then get back (by link or browser “back” button) to previous page, it now works there
I admit this is strange. It looks almost like for some reason initially some JS scripts were not loaded or executed properly – which is unexpected.
However, it’s also not impossible. I asked you previously if you are using some sort of JS optimization tools on site but now I noticed also that you’re using DIVI so could you check following options in “Divi -> Theme Options -> Performace”?:
1. Defer jQuery and jQuery Migrate
If it’s enabled see if it also has “Defer Additional Third Party Scripts” option enabled – if so, try disabling it.
If it’s enabled but doesn’t have “Defer Additional Third Party Scripts” option enabled – disable entire “Defer jQuery and JQuery Migrate” option.
Clear all caches (including clicking “Clear” option next to “Static CSS File Generation” on “Divi -> Theme Options -> Builder -> Advanced” page) and check if the issue is still happening.
If it does, then
2. “Dynamic JavaScript Libraries” – try temporarily disabling that option and se if it helps
3. and if still nothing – try disabling the “Dynamic Module Framework” option all together

If step 1 solves the issue, it would be recommended to keep it set that way.
If step 2 doesn’t solve it but any of steps 2 or 3 does, we’ll look into it closer to see if we can somehow mitigate it other ways (but this will need to be checked by our developers).
