The form does not display on page

    Hi! I have created a form in the page of confirmation of the newsletter subscription, for clients to send me their preferences after they have suscribed. The form is created, all set and should appear below of the text.
    I don’t know why is not showing up. I have completed all information and steps and it is activated.
