The form is not working

I don’t know why but the form is not working. When I try it is said that server is not working. I asked to the server support, and they told me that there was a problem with PHP autentication. So I added a plugin for that, but the problem carry on.
So I don’t know what else to do. Maybe I am filling the form wrongly…But I don’t think so…
