The map always disappears

I have been tried in both localhost and live site. The map shows only few days at the beginning and then has gone. I tried many times in configuring API key, Initial Zoom Level, Content for Info Window as there are no hints for these errors. I even checked all the address in Google and tested with the map one by one with the plugin. However, the maps disappear again few days later. Time wasting and exhausting!!!
