Yes the requirement for cache plugins is mentioned about halfway down the homepage:

Where it says:
“Using a caching plugin? Check out the Installation notes for important info.”
And then there is tons more information about what this plugin can and can’t do, requirements, cache plugin infos, and everything else on the linked Installation page.
So the plugin lets people know clearly about the “no page cache” requirement, including several mentions elsewhere in the documentation, FAQs, Help tab, and so forth.
Bottom line: If your site needs a page-cache plugin, then Blackhole is not for you. For all other sites, Blackhole is a great way to stop bad bots (as evidenced by over 10K happy users).
