The SEO title not working as expected

Why is it when I enter meta data into individual pages the browser’s title gets overwritten by the SEO title. It only shows the SEO title and no longer show this format – %%title%% %%page%% %%sep%% %%sitename%% ? And if i leave it empty it shows titles correctly? I tried Force rewrite – didn’t help…
Please help!
