theme my login custom edit profile page

hello everybody,

I am trying to create a custom template page for update profile.

My current one has multi address lines that are the same.
This issue is caused by another plugin wlm.
My goal is to remove one set of address fields.
or hide them and add echo field to fill in the details.
also I would like to add avatar uploading for contributors
I have avatar access for admin,author and editors.
