It does not seem that my plugin is properly installed and enabled.
Try disabling before wp-super-cache, wait for the cache is emptied at least 1 hour or more, and then activate my plugin.
Also try to disable all plugins except WP Admin Bar Removal
Also try to apply WordPress Default Theme!
After contact me again and wait for me to check if my plugin is installed correctly!
Typically Admin Bar is not available unless you are logged in, then there is a customization that causes this problem, and certainly not my plugin which apparently is not even activated.
WP Admin Bar Removal is compatible up to WordPress 3.7+ for now, but work also on WordPress 3.8+ and 3.9+ without any guarantee!
Remember that practice to disable Admin Bar would not be allowed, so take what comes.
