This Plug in Doesn't Work for Me

I cannot seem to enable ANY comment system on my website. My Jetpack comments don’t work either. I have tried Disqus, CommentLuv, WPDiscuz and now I’m trying this Comments Evolved for WordPress site. I would love to use the plugin but after trying to configure it, it still doesn’t show anywhere on my site. My theme’s PHP (in the commennts PHP page, includes the required comment_form () that is supposed to be in the code.

***PLEASE HELP*** Without a commenting option there’s no point in having this website.
