tiny avatar in internet browser bar

hi,

so i was able to code my page so that a tiny avatar shows up in the internet browser bar, but it only works on the index page strangely? I put the code in the header.php file, thinking that that would populate in each page, but it is not working that way.

how can i get it to show up on each page of my blog and not just the index page?
