TinyMCE Advanced: Can’t select some formats

I have several styles registered but only two of them can be selected for some reason. See here

Here are my functions hooking into ‘tiny_mce_before_init’ and ‘mce_buttons_2’.

Here is my editor-style.css file.

There is one javascript error showing in the console. Not sure if that’s related.
