TinyMCE not adding image tags

As far as I could search this is a new issue.

I uploaded an image to wordpress and whenever I try to add it to the post, nothing happens. The same thing was happening to galleries until I manually entered the shortcode [gallery].

Can anyone please point me to the right direction on how to troubleshoot this issue?
