first thank You for this great plugin ! I love it ??

Here are few tips from me:

1. in settings page, at the bottom is typo “Disabe metabox”

2. can you in the same settings add few more words to people who don’t know what is metabox? like “Check if you don’t want to display the metabox when editing a post or a page.”

3. In “Active Fields” and “Required Fields” what is the field “Reason” ? I don’t see field with that name in the form itself. Maybe you can remove it, as it is unchangeable anyway?

4. PLEASE if possible add field “Issues” in the “Active Fields” and “Required Fields”, so we can hide it (I’m not using it) and even when I delete “Issues” field in Settings, the field is still in the form, but empty.

5. Can we have in the form just little bigger text field Details or option to change its size in Settings?

6. Can we have option to customize more the button for opening form? Like I want it just as text hyperlink, not rectangle, have an icon before the button text etc. Sure I can change it somewhere in css, but something more elegant in the Settings page.
