Can you please let us know what plugins and theme do you use?
You can attach your install info file which you can find in Ultimate member -> Settings -> Install info tab.
Please try to disable all plugins except Ultimate member and JQuery Updater and switch to the default WordPress theme and see if you can replicate this issue.
