Token Expired, Not Posting to Facebook, App hidden to all non-admins

The first issue we are having with the app is that we repeatedly get the alert:
Your Facebook Access Token for WPBook has expired. Please visit the settings page for WPBook and grant a new access token. Until you do so, cross-posting to Facebook and import of comments will fail.
In Settings I click the “checking permissions” link then back-up and press Save, and it clears for a few hours. What do we need to do to get this alert to go away for good.
The second issue is that despite having it set to publish new posts to the (OICmoments) Facebook wall, it does not do so.
The third issue is that having created an FB app and having followed their “detailed instructions” to the letter, the corresponding tab on our FB page is not visible to the public and the contents blank to all but admins.
