your theme is adding additional padding to some elements. To fix this, simply add the following code to your Custom CSS field, under Form Options in Optin Forms:

#optinforms-form1-title,#optinforms-form1-subtitle,#optinforms-form1-disclaimer{padding:0!important;}

The second issue is also due to theme’s CSS. Please add the following code to your Custom CSS field:

#optinforms-form1-name-field,#optinforms-form1-email-field{height:34px!important;}#optinforms-form1-disclaimer{margin:15px 0!important;}
