transfer to sheet: only 1st column get the data

Hi,
I’ve created a form with 2 questions (text).
when I use the live form (https://docs.google.com/forms/d/1pRCcWRHTrhIY5ykGJj4rSZc_N0FAUkFddzLMZdjNhzM/viewform) everything works as expected – the content of question1 + question2 get transfered to the sheet.BUT when I use the same form via wordpress plugin, the content of question 2 did not get transfered to the sheet – the column will be blank but the timestamp shows that an action was made!?

do you know why the 2nd column did not recieve the data and what the problem is?
