Trouble clearing cache

I’m having a hard time clearing the cache after publishing a post and after changing minor content, like images. Empty All Caches does nothing and emptying the browser cache does nothing.

After a post is published, the home page does not show that new post (and I appear logged out). I refresh the page and now the new post appears (and I’m logged in).

Also, when uploading a change to an image or other simple content, clearing the cache will not display the new content. The only way to see the change is to deactivate W3TC, view the change, then re-activate W3TC.
