Ultimate Membership Pro + other plug-ins

I currently have Ultimate Membership Pro and Captain Form (123Frombuilder) and need to know the following:
1a) I am creating a form that I want my subscribers to fill out online and that information need to be available to them whenever they log in. Can I do this using my Bluehost/WordPress site? 1b) How do I set up the form so that each subscription has its own separate file in the database or will my subscription add on do this automatically?
