Unable to add forms

    I am unable to add forms to my site. I have tried Smart Forms, Visual Form Builder Pro, Form Maker, and a few others. Smart Forms isn’t even adding the short code, but all the others give me a blank page in the body. I am using Catch Themes Kathmandu Pro. I switched to the Twenty-Eleven Theme and the same thing is happening. I have also deactivated all of my other plugins and the issue continues. Any idea what is going on? The url to my site is

    I resolved the issue. I have a Honey Pot set up. I realized that the short code for the forms was being saved in the middle of the hidden honey pot code. I removed it and clearly separated the form short code from the honey pot code and the form now shows up.
