Unable to get Form Fields to list vertically

Unfortunately I have the same problem over here!
Judging from the various posts in this Support Forum with no replies to them (some of them ask the very same question as you do) it seems to me that Richard has lost focus on his project. That’s a real pitty, since this is a great plugin. It just needs some minor enhancements which are requested here.

I got this to work. Not the best result, but it is vertical:

Last Name:
