Unable to save file to category

Hello,
We are able to upload a file just fine, however, it will not save it to the category we select. After save, it goes back to having the category as “blank”. Even if we then click edit file, and then select a category, it still won’t save. Any ideas?? We’ve tried deactivating all other plugins, still does the same thing.
