Unable to view images

Upon a user registering on our website, we want to go through a review process. Part of the registration is uploading an image for a profile picture, and a separate for a signature.

When we go to review the info, we get a link to the image, but it does not work, showing that the page does not exist. This issue persists even after accepting said user.

What is the issue here please?
