Unexpected Server Error

I’m running this plugin on two different sites. The first one works great. The second is generating an error.
On the first site I’m using a version of the Fable theme. On the second I’m using the Academy theme. The error displayed is:
Unexpected server response (0) while retrieving PDF “https://www.larkacademic.com/wp-content/uploads/2015/06/Placeholder.pdf”.
NOTE this site is actually installed in a subdomain of “https://academy.larkacademic.com/…” not in the root as the error shows…
