Unfortunately the plugin seem currently not to work

Hi support team,

I’ve double checked and found out that the plugin seems not to work right now.
1. I’ve setup and added the code

2. then I’ve tried. I get an form submision email, but no accept email from mailpoet for add new member to the list.

If I use the original form from mailpoet, it’s working, but if I use your plugin within my subscrition form then its not working.

Please help to find out why and how to solve.
