unlisted video in my list

i think somehow now it shuffles on and off a bit but has been persistently present in the selected four in the past and now when i am logged into wordpress, it is almost always present but when i log out, it occasionally come up. That video is not in any of my lists, uploads or likes.
see thumbnail at lower left of the second image and that is it.
in the first image, i opened it in the lightbox

After refreshing page a lot of times, I spot that video. You have randomly displayed videos in that widget from playlist https://www.youtube.com/playlist?list=PLB343CD3C7F2D37E4

First video in that playlist is https://www.youtube.com/watch?v=67VZ-Dn-I9

So, edit playlist and remove that video from there.
