Unneeded Account Logout Prompt

Hello,

When you are trying to log out from the user account, you get a new page where you are prompted if you really want to log out.
Not only this is unnecessary (even ugly I must say), but also poses a big sec. issue when you are trying to hide the WP-Login page using plugins like this, as it discloses the admin page path…

How it should work?
When you click on Logout, you get logged out and redirected to either the home page or the mywebsite.com/account/ page (you choose).
