Where can we see the website in question?

To clarify, I see these extra contact forms when I look at settings for Contact Form 7 in the WP dashboard.

Plugins –
Slider Revolution, Yoast, TablePress, I Recommend This, Akismet

Try switching to the default theme and deactivating all plugins excluding Contact Form 7, Akismet, and Yoast SEO.

Why you are advised to deactivate plugins and switch to the default theme.
