Unwanted extra radio buttons

    I have a survey and there is one extra unwanted radio buttons displaying as part of the multiple answers as shown in the link https://suppliss.com/suppliss-survey/
    Also the question numbers are are displaying exactly in front of the questions.
    Kindly help me to rectify this issues.

    To help check whether there is script conflict with your active theme, try temporarily switching to one of default WordPress theme.
