Upgrade documentation revised, please feedback

I’ve done quite a number of upgrades lastly and more or less hated the instructions. Which is why I’ve revised them today.

Can I ask you to help me check them for correctness and completeness? The suggested new version can be found at the respective discussion page in codex. Please leave feedback in comments over here.
