Upload button not showing up

I just installed WordPress, BudyPress and rtMedia. I create a page and added the image upload shortcode as follows: [rtmedia_uploader] I do see the upload box, but it is missing the ‘Upload’ button. Even after selecting some images, there is no Upload button – see screenshot:

I have tried to adjust the shortcode by parsing a context, etc, but that didn’t help. FYI, am using the default WP TwentyTwelve theme.
Any suggestions?
