Upload/Browse image attachments not displaying

After upgrading to the latest version of WordPress, adding images to posts no longer works.

When uploading a new attachment, or browsing to an existing attachment, no thumbnail displays. Instead, the title for the image is displayed.

Basically, images are being treated like text attachments.
