Uploaded image titles being converted to question marks?

Hi,

I’m experiencing a weird issue when uploading images. Some images are having their titles converted to question marks, as seen in this screeshot:

Has anyone seen this before? I’ve been searching the net for answers, but I’ve had no luck yet.

This is only happening on SOME images, not all.
