Upload/Insert buttons failing

Very strange, but my upload/insert buttons aren’t working. Actually a few minutes ago, I think they did, but it’s very intermittent functionality. I tried in Chrome and Firefox. I’ve restarted the computer, checked the click everywhere else. . . is there something I can look into to make sure some code didn’t get corrupted?
