User form

I would like to provide the members of our group with a “Personal Information” form that they could control. Each member would fill in only the information they would like to share with the other members. They would be able to log on to the site and edit the information when ever they wished. The form would be part of their membership information and be reachable from a link on their gallery page.
I know this must fairly simple but my mind has gone blank.
