User generated data

I’m looking for a way to allow my registered users to record their volunteer hours (name, location, hours worked, comments) via a simple form. As an incentive to get folks to fill this form out, I’d also like to be able to display information from the volunteer log on my site.

Seems like most form plugins out there are aimed at collecting information, but not displaying it. Any thoughts/suggestions on how to do this? My php/sql skills are very limited so I’m trying to find an existing plugin to accomplish this.
