Using default theme and diabled plugins, still get scrolling arrows

Hello,
I’m using this plugin to run a questionnaire on my site (corpath.org/questionnaire). Until recently, the plugin worked great, but now the plugin has ceased to work. I always get the scrolling arrows. I’ve tried disabling all of my plugins and I use the Twenty Twelve theme. Any suggestions?
