Using WP 3.8 – How to change input fields?

This might just be staring me in my face but i can’t seem to find it.
Is there a simple way to change the default fields in the review form?
Currently the fields are as below. But i’d like to add more fields to this.Name: *
Email: *
Website:
Review Title:
Rating:
12345
Review:
