UTM Generator

  • Hello,

    Does anyone know of a plugin that I can put on my website that acts like this,

    So people would come to my pages and fill this out to get a URL with UTM?

    If not anywhere you know of some easy code?

    I tried this code but it broke in the last 2 weeks.
