Version 4.0.4. breaks upload functionality

    After updating Download Monitor on a website to the latest version 4.0.4 the plugin causes a bug which prevents adding files. I installed a clean, empty new wordpress install locally with only the Download Monitor installed and can confirm this happens.

    The problem in detail:
    – Go to Downloads > Add new
    – Try clicking on the Downloadable Files/Versions block. The ‘Add file’ button is not visible. The block doesn’t expand.

    Installing the previous version 4.0.3 solves this problem
