    Works fine. We use this form because it has the option of a follow-up page, so we can easily use google adwords tracking.

    One thing that could be better: CSS was hard to edit. Finally edited the form’s own CSS because I couldn’t seem to overrule it otherwise(?!)

    You’re welcome, and thank you for the plugin.

    To be more specific: Some CSS could be edited in the usual way, but the spaces between the text fields wouldn’t be fine-tuned, it seemed. I also couldn’t find out a common denominator. On the other hand: You can’t please everyone. This is a simple form and I think it’s best kept simple. Maybe simplify the CSS a bit more too, that’s all.
