Video producing duplicate images?

Hi, I’ve got a serious problem!

The feature video I put in has recently started producing multiple duplicate feature images. This happened with the last couple of videos I tried to use as feature video thumbnail (live). It will make hundreds of copies of the same video feature image taken from the video I put in. I’ve had to go back to delete thousands of copies, and now the media library is still reading these files except they show up as broken now.

So there are two issues I’d like to fix:

-Stop the video plugin from producing duplicate images from a feature video
-figure out how to remove these copies that aren’t supposed to exist anymore from the media library (I deleted them via FTP but WordPress is still reading these files and showing up as broken images)..

Any kind of help would be greatly appreciated. If it helps any, I’m also running WPMU, the only plugin capable of making duplicate copies for other languages, but it’s been working fine for months with the video plugin, so I’m not sure what’s changed recently.
