Video Thumbnails admin page not loading only blank white screen!!!

I had this plug-in working beatifully until recently when i selected the plug-in options link from the Dashboard and only got a blank white screen.

The plug-n widget still works in the edit post/page admin page. But i cant run the script from the options page in the Dashboard.
