Virtual product with questionnaire AND scheduling

Hi… Help!
I am creating a coaching site where one of my products is individual coaching sessions. Costumer will purchase the coaching session and then they have to choose an appointment and fill out a form prior to the session.

I cannot wrap my head around this. What is the best way to do this? I am using the premium virtue template and I have woo commerce installed.

I am thinking that either a unique email template is sent to the costumer when this specific product is purchased that will take them to their account page where they would fill out a form and choose the date.

Another option is to have the whole process embed in the checkout process – though not on the actual product page. This option seems ‘cleaner’. But is it possible?
