Visual Editor broken after upgrade to 3.0.1

After upgrading to 3.0.1 the visual editor is broken in Firefox, IE, Safari, Chrome on both OSX and Windows.

When I go to create a new post, the editor loads, you see the visual editor toolbar and then it disappears. When I click on HTML editor, the toolbar for the HTML editor loads no problem. I click back over to Visual and the toolbar disappears but the HTML stays.

I’ve tried disabling all plugins, no luck.
Tried changing the theme to the default theme, no luck.

Not sure what else to try. Please help!
