Sudden "Sorry, that item was not found" Error

I’ve a client who has been using VoucherPress for a number of months without problems. Now, without any updates to my WP 3.4.2 install, a new voucher does not work and gives a “Sorry, that item was not found” error on the resulting page when the voucher link is clicked. Any ideas on what the issue is?

The voucher setup details are simple (and same as previous working voucher):
– Custom image background template with basic text entered in fields
– Dates for when vouchers starts and ends (set to 20140129 – 20140601)
– Voucher available: checked
– Use sequential codesI tried a few different settings like using random and single custom codes. Also, ensured the dates aren’t overlapping from previous voucher. Confirmed I’m using the right link URL and shortcode.
One issue that comes to mind is the query string name value pair URL structure that calls from the root domain. Is there maybe a permissions or security or updated PHP issue that the host could have initiated? This is out of my depth but it seems rare for a plugin to use this URL structure.
