W3 Total Cache error:It appears Minify URL rewriting is not working

After upgrading to the latest W3 Total Cache version, I get the following error:
W3 Total Cache error: It appears Page Cache URL rewriting is not working. Please verify that the server configuration allows .htaccess
Unfortunately disk enhanced page caching will not function without custom rewrite rules. Please ask your server administrator for assistance. Also refer to the install page for the rules for your server.I have googled and found that other people have had these problems too (years ago, but also recently). I could not find a solution that worked for me though.
Is this a bug that will be fixed in a newer version? Or do I have to change some settings?
W3 Total Cache Version: 0.9.4.1
Also, on a WooCommerce product page with variations, after upgrading I suddenly saw weird comments like “/*]]>*/”.
This was resolved by disabling Minify -> HTML minify settings -> Inline JS minification. I am not sure if the problems are related.
