Want to design online test

Hi,

I want to design online test slimier to given link. Below things is mandatory for my test.

1. Each page has 2 question and top pages progress highlight.
2. After completing all question ask user information(Name,Email,Company etc)
3. Once it submit it should show result in a graph or chart.
4. Email notification

Please suggest me any plugin which has these functionality.
