Website terrible slow, Perf. grade 94/100

Lately, since a week or two, our website is having a serious issue. With an average load time of 12,61 sec, we have urgent need to speed up our website.
We are currently using Salient’s theme and a couple of plug-ins including W3 Total Cache (see full list below). I have tried to optimize our database (went from 60 Mb down to 40 Mb), deactivated all plug-ins, tried other themes, reinstalled WordPress and went from WordPress Super Cache to W3 Total Cache. None of them improved our loading speeds.
Other WordPress websites on our VPS loads within 631ms. You say what? Yes, indeed, 631ms according to GTMetrix. Our server specs: dual core Xeon (10% CPU load on average), 4 GB ram, cPanel (NGINX instead of Apache, XCache, ModSecurity, DSO handler), total websites: 5 WordPress installations with (an average) of 1.400 visitors a day. That shouldn’t really be the main problem at the moment.
Our error_log is for example only 2kb (created at 10-01-2015 and our .htaccess has been entirely rebuild from the bottom to the top. That being said, I have no clue what’s left.
